After hearing the words “You have pancreatic cancer,” many patients naturally assume the next step is treatment.
In reality, the next step is almost always understanding the disease as completely as possible. That process begins with imaging.
For most patients with pancreatic ductal adenocarcinoma (PDAC), the single most important imaging study is a pancreas-protocol CT scan of the abdomen and pelvis. This examination is the foundation upon which nearly every treatment recommendation is built.
Why imaging comes before treatment
Before recommending surgery, chemotherapy, or radiation therapy, your physicians need to answer several critical questions:
- Is the tumor confined to the pancreas?
- Has it spread to the liver, lungs, or elsewhere in the body?
- Is it involving the major arteries or veins next to the pancreas?
- If surgery is possible, can the tumor be removed safely and completely?

These questions cannot be answered by a biopsy alone. They require high-quality imaging.
Not all CT scans are the same
Many patients are surprised to learn that there is more than one type of CT scan.
A CT scan performed in an emergency department may be excellent for diagnosing appendicitis, kidney stones, or bowel obstruction. It may even identify a mass within the pancreas. However, identifying a tumor and planning treatment are two very different goals.
A pancreas-protocol CT scan is specifically designed to evaluate pancreatic tumors. It uses carefully timed intravenous contrast and thin-section imaging to produce detailed pictures of the pancreas and the blood vessels that surround it. These images allow physicians to determine not only that a tumor is present, but also its precise relationship to nearby arteries and veins.
Whenever possible, staging should be performed using a dedicated pancreas-protocol CT interpreted by radiologists experienced in pancreatic diseases.
Why blood vessels matter

When reviewing your CT scan, your physicians are not simply asking, “Is there a tumor?” They are asking much more detailed questions:
- Is the tumor touching the superior mesenteric vein?
- If so, how much of the vein is involved?
- Is the portal vein narrowed or blocked?
- Does the tumor contact the superior mesenteric artery?
- Is there enough normal blood vessel to allow safe reconstruction if surgery is performed?
The answers to these questions help determine whether surgery should occur immediately, after chemotherapy, or not at the time of diagnosis.
Looking for cancer beyond the pancreas
A pancreas-protocol CT also helps physicians determine whether the cancer has spread outside the pancreas. The liver is the most common site of distant spread, but physicians also carefully examine the lining of the abdomen (the peritoneum), nearby lymph nodes, and other abdominal organs.
All patients should also undergo a CT scan of the chest because the lungs are another potential site of metastatic disease. Identifying cancer outside the pancreas before treatment begins is essential because it may change the recommended treatment strategy.
Occasionally, additional imaging is recommended. An MRI of the liver may help clarify small abnormalities seen on CT. In selected situations, other specialized imaging studies may also be appropriate. Each test answers a different question — together, they provide the most complete picture of the disease.
Imaging does more than determine whether surgery is possible
Patients often think imaging has one purpose: deciding whether the tumor can be removed. In reality, imaging influences nearly every aspect of treatment planning. It helps physicians:
- Determine the clinical stage of the cancer.
- Assess whether chemotherapy should be given before surgery.
- Plan the technical details of an operation.
- Identify patients who may benefit from vascular resection and reconstruction.
- Evaluate whether radiation therapy may play a role.
- Establish a baseline for measuring how the tumor responds to treatment.
As treatment progresses, repeat imaging becomes equally important. Follow-up CT scans allow physicians to determine whether chemotherapy is controlling the disease, whether new areas of cancer have appeared, and whether surgery has become possible after treatment.
Where you have your CT scan can matter
Because pancreatic cancer is relatively uncommon and technically complex, not every imaging center performs pancreas-protocol CT scans routinely, and not every radiologist interprets pancreatic imaging on a daily basis.
Whenever possible, imaging should be obtained — or at least reviewed — at a center with expertise in pancreatic diseases. Experienced pancreatic radiologists are trained to recognize subtle findings that can influence treatment decisions, including small liver metastases, the degree of vascular involvement, and anatomic variations that may affect surgery.
Many multidisciplinary pancreas programs routinely re-review imaging that was obtained elsewhere. It is not unusual for additional findings or different interpretations to emerge when studies are reviewed by specialists who focus on pancreatic cancer. This does not mean the original scan was performed incorrectly — rather, it reflects the value of specialized experience in evaluating a complex disease.
